Acclaim Rehab and Nursing Center is seeking a dependable and compassionate

Home Health Aide (HHA) to assist residents during clinic and dialysis appointments while ensuring their safety, comfort, and well-being.

Key Responsibilities

• Escort residents to and from clinic and dialysis appointments
• Arrive one hour prior to scheduled appointments
• Assist residents in preparing for appointments
• Ensure residents receive snacks and required documentation
• Monitor residents to prevent falls or incidents
• Report all incidents immediately to nursing leadership
• Communicate with clinic staff and facility regarding resident status
• Maintain professionalism and follow all facility policies

Qualifications

• Current New York State HHA certification (required)
• High school diploma or equivalent (preferred)
• Strong communication skills and reliability
• Nursing home experience (preferred)
