Position Overview

Seeking a

Physical Therapist (PT)

to provide home health care, enabling patients to recover in their own homes. Responsibilities include conducting initial and ongoing assessments of mobility, strength, and function; developing and implementing personalized treatment plans; delivering therapy services per physician orders; documenting progress and modifying goals; and managing discharge planning with patient education on home exercises. The role involves collaborating with healthcare teams, supervising personnel (PTA, HHA), and completing assessments like OASIS where permitted. Adheres to policies, standards, licensure regulations, and state laws, ensuring effective communication with staff, patients, and families.

Qualifications & Skills

• Degree in Physical Therapy from an accredited program
• Current, unrestricted state license
• Valid CPR, driver’s license, and proof of auto insurance
• Preferred: strong organization, communication, attention to detail, time management, relationship-building, and home health experience
